As the State’s COVID-19 vaccine program transitions to regional vaccination sites, we are working toward completing the local efforts to vaccinate our residents age 75 or older. If you or someone you know is at least 75 years old, a resident of Jamestown and has not been vaccinated nor has an appointment for a vaccine, please contact the COVID-19 call center at 423-1213 Ext 4328. UNABLE TO LEAVE YOUR HOME FOR A VACCINE: If you are unable to leave your home to receive vaccine, or if you care for someone who cannot leave home, complete the form at to let the Department of Health know.
